Multiple Teams · Sports Physicals for next school year will be offered on April 18th


Dear Richland Northeast Parents and Guardians,

It is once again time to start planning for pre-participation sports physicals for your student athletes for the 2015-2016 school year.  On Saturday, April 18th, 2015, the Moore Center for Orthopedics will be hosting its annual Physical Day at their location in Northeast Columbia.  All high school and middle school students interested in playing sports at Richland Northeast next school year are encouraged to attend.  Multiple high schools in the area will be attending physical day, so the Moore Center has scheduled Richland Northeast High School for the 8:00 am time slot, to allow our athletes to be seen in a timely manner and to accommodate athletes who have competitions that afternoon.

Students and parents/guardians will be required to complete the physical packet in its entirety prior to physical day.   Paperwork includes a checklist for all school-required forms, the SCHSL-Approved Physical Form, the Richland Two Sports Health Form, and Concussion Information for Athletes and Parents.  Please review and complete all sides of all pages in the packet.  The medical history on the physical form should be completed in its entirety and all forms MUST be completed and signed by the student and parent/guardian or the student will not be eligible to receive a physical.  The cost for a physical is $10 and you may pay with cash or a check payable to “Richland Northeast High School”.  All paperwork and money must be completed by Wednesday, April 15th and turned in to Athletic Trainer Kate Benedict after school in the athletic training room.  The main office will not be accepting any money and/or paperwork at any time.  No paperwork will be accepted after April 15th and no money will be collected at the school or at the Moore Center on the day of physicals.  When a student turns in all forms and money, he or she will be added to the roster for Physical Day.  Bus transportation from RNE to the clinic is provided and the bus will leave RNE at 7:30 am that morning.  NO student will be permitted to ride the bus if forms and/or money are not turned in.  In order to guarantee your child has a seat on the bus, all forms and money should be turned in by Monday, April 13thAfter that time, we cannot guarantee transportation for your child to and from the clinic.

Make-up physicals will not be offered by the Moore Center for Orthopedics.  If you do not take advantage of this opportunity, it will be the responsibility of the student and parent/guardian to obtain a physical prior to the start of their team’s first practice.  As a reminder, physicals must be dated on or after April 1st, 2015 to be valid for the 2015-2016 school year and the SCHSL-approved physical form is the only physical form that will be accepted.  All forms in the packet must be on file with the Richland Northeast Athletic Department before participation in pre-season and in-season practices and conditioning.  Physicals and sports health forms from the 2014-2015 school year are no longer valid after June 30th, 2015.  If you have any questions, you can contact our athletic trainer (Kate Benedict) at kbenedict@richland2.org or by phone at 803-699-2800 ext. 79867.
